Kidney Failure Risk Calculator

Estimate 2- and 5-year kidney failure risk using the four-variable Kidney Failure Risk Equation (KFRE).

The four-variable Kidney Failure Risk Equation estimates the probability of treated kidney failure—dialysis or kidney transplantation—over the next 2 and 5 years in adults with chronic kidney disease stages G3–G5. The equation uses four routinely available clinical variables: age, sex, eGFR, and urine albumin-to-creatinine ratio (ACR).

Calculate Kidney Failure Risk

Enter the four variables used in the published Kidney Failure Risk Equation. This calculator uses the North American calibration.

Enter age in years.
Sex variable as specified in the published KFRE.
Intended for established CKD stages G3–G5 (eGFR below 60).
Enter a value greater than zero.
Choose the unit reported by the laboratory.

Estimated Kidney Failure Risk

2-Year Risk —
5-Year Risk —

These percentages estimate the probability of treated kidney failure (dialysis or kidney transplantation) within the specified time period. They are population-derived estimates, not a prediction of exactly what will happen to an individual patient.

Model: Four-variable Kidney Failure Risk Equation using age, sex, eGFR, and urine ACR with North American calibration. This calculator is intended for adults with established CKD stages G3–G5 who are not already receiving dialysis or living with a kidney transplant.
