Home BP Tracker v1.9

Home Blood Pressure Tracker

Look beyond one number. Understand your patterns and prepare for your kidney care visit.

How to measure at home

For seven days, take two readings one minute apart each morning and evening, unless your care team advises otherwise.

  • Use a validated, correctly sized upper-arm cuff.
  • Avoid exercise, caffeine, and smoking for 30 minutes; empty your bladder.
  • Sit quietly for five minutes with feet flat, back supported, arm at heart level. Do not talk.
  • Record every reading and any relevant symptoms or medication timing.

Understanding home blood pressure goals

Home readings are useful for identifying patterns, but a single reading does not diagnose hypertension or determine treatment. The values below are general reference points, not personalized prescriptions.

135/85 mmHg — common home assessment threshold

An average at or above this level is commonly used to identify elevated home blood pressure in some guidelines. It is not a universal treatment goal.

130/80 mmHg — a frequently used treatment goal

Some U.S. guidelines use a goal below 130/80 mmHg for many adults with hypertension, including those with CKD, when appropriate and tolerated. This is not interchangeable with the home diagnostic threshold.

CKD and albuminuria

Albuminuria (urine ACR ≥30 mg/g), diabetes, cardiovascular risk, frailty, orthostatic symptoms, and medication tolerance may affect how intensively BP is managed. There is no single home target determined solely by albuminuria.

Important measurement distinction

Some kidney guidelines discuss a standardized office systolic target below 120 mmHg when tolerated. That target must not be applied directly to routine home readings or nonstandardized office measurements.

Discuss your individual target with your primary care clinician or nephrologist. Ask which home average to aim for, whether to exclude the first day's readings, and when to call about high or low readings. Do not change medication based on this tool.

Saving your readings

Your readings are saved in this browser on this device. You can return to this page later using the same device and browser to continue tracking. No account is required, and readings are not automatically sent to Integrative Kidney Institute.

To avoid losing your history, do not use private/incognito mode or clear this website’s browser data. Readings do not automatically sync to another device. Browser storage can still be deleted or unavailable, so export a CSV backup regularly. You can import that backup later or on another device. Anyone with access to this browser profile may be able to see the readings.
