Mayo Imaging Classification in ADPKD


Total kidney volume is an important imaging-based marker of disease severity in autosomal dominant polycystic kidney disease (ADPKD).

The Mayo Imaging Classification combines age with height-adjusted total kidney volume (htTKV) to classify patients with typical ADPKD into imaging classes 1A through 1E. Higher imaging classes are associated with progressively greater rates of kidney growth and a higher likelihood of more rapid kidney-function decline.

This calculator is intended for patients with typical Class 1 ADPKD morphology. The Mayo Imaging Classification should not be applied as a prognostic tool to atypical Class 2 imaging patterns.

Enter the patient’s age, height, and right and left kidney dimensions. The calculator will estimate total kidney volume using the ellipsoid method. The calculator will then determine height-adjusted total kidney volume (htTKV) and assign the corresponding Mayo Imaging Class for typical ADPKD.

Interpreting Mayo Imaging Classification


The Mayo Imaging Classification divides typical ADPKD into five imaging subclasses based on age and height-adjusted total kidney volume.

1A — theoretical annual kidney-volume growth less than 1.5%

1B — approximately 1.5% to 3.0% annual growth

1C — approximately 3.0% to 4.5% annual growth

1D — approximately 4.5% to 6.0% annual growth

1E — greater than 6.0% annual growth

Increasing Mayo Imaging Class is associated with progressively greater risk of kidney-function decline. Classes 1C, 1D, and 1E may indicate more rapidly progressive disease, but imaging classification should be interpreted together with eGFR trajectory, genotype when available, clinical history, age, comorbidities, and other prognostic information.

Method: Each kidney volume is estimated with the ellipsoid formula: π/6 × length × width × depth. TKV is the sum of both kidney volumes; htTKV is TKV divided by height in meters.

Important: Mayo subclasses 1A–1E apply only to typical Class 1 ADPKD. Atypical Class 2 morphology requires separate clinical assessment. Estimated volume from linear measurements is less precise than stereology or planimetry. This educational tool does not replace radiology review or clinical judgment.

Important Limitations


The Mayo Imaging Classification is intended for typical Class 1 ADPKD morphology. It should not be used to predict progression in patients with atypical Class 2 imaging patterns.

Accurate TKV measurement is important. MRI- or CT-based automated or semiautomated segmentation is preferred when available. TKV estimated from kidney dimensions using the ellipsoid method is less precise and should be interpreted accordingly.

Mayo Imaging Classification is one component of ADPKD prognostic assessment and should not be interpreted in isolation.
